A statutory declaration is a solemn declaration of fact or belief, and may generally be made in relation to any matter.
You are responsible for the content of the Statutory Declaration. If you are unsure of what you are required to write on the Statutory Declaration, you should contact the authority who has requested that you sign one.
The Statutory Declaration must be signed in front of the Consular or Diplomatic Officer and cannot be witnessed by mail. You will be required to present current photo ID, showing both your current name and signature.
You are responsible for forwarding the Statutory Declaration onto the relevant authority.

• the parties are not closely related

You MUST follow the guidelines contained in that link when lodging your application by mail, incomplete applications will cause delays.
If you dont meet the criteria, you'll need to lodge your application in person by appointment only . You should allow a minimum of 6-8 weeks to get a new passport
